Archaeology Site Forms
This section of the Wisconsin Office of State Archaeology's Web site contains information on completing Wisconsin's Archaeology Site Inventory form, the Burial Sites Report Form /Report of Disturbance form, and the Bibliography of Archaeological Reports Form (BAR). Each of the forms is available in a PDF and in a WORD fill-in document. Archaeological site forms should be submitted prior to sending the report so that the state numbers can be incorporated into the text. Please submit the forms to John H. Broihahn. We generally respond within one working day.
BAR forms should be included in the report. These forms facilitate report processing and WisAHRD updates.
Forms for non-archaeological burial sites—these generally include marked, maintained, post-contact period burial areas—should be submitted to John Broihahn.
