Cruciform plan on poured concrete foundation. The main facade faces east onto CTH D. All of this building's door openings have dressed stone lintels and stone sills. In addition, almost all of the building's window openings retain their original one-over-one double hung wood sash windows and the building's original main entrance and its side and transom lights are also still intact.

CENTRAL 2-1/2 STORY GABLED MAIN BLOCK W/WOOD SHINGLED GABLE. 1-1/2 STORY HIP-ROOF SYMMETRICAL SIDE WINGS, SMALL WOOD SHINGLED GABLET W/SMALL WINDOW IN EACH WING. PAIRED WINDOWS W/CONCRETE LINTELS, SEGMENTAL ARCH WINDOW IN FRONT GABLE. TRANSOM OVER ENTRANCE.

2011- "This previously surveyed cruciform plan building is located on the northwest comer that was formed by the intersection of CTH D and USH 8 and it was built in 1912 to house a three-room State Graded School. The building has poured concrete foundation walls and the exterior walls that rest on this foundation are clad in red brick, and these walls are sheltered by the overhanging eaves of the hipped roofs that shelter the building's three parts. The central portion of the building consists of a rectilinear plan two-story-tall main block that is sheltered by a gable-and-hip roof whose ridgeline runs east-west and whose steep slopes are clad in deteriorating asphalt shingles while one-story-tall hipped roof rectilinear plan wings whose ridge lines run north-south are attached to both the wider north and south-facing side elevations of the central portion. The symmetrically designed main facade of the building faces east onto CTH D and the first story of the center portion contains a deeply inset main entrance, above which is placed a nearly full-width dressed stone name plaque that reads "State Graded School." Placed in the center of the second story above this plaque is a single flat-arched window opening that still retains its original large four-light wood sash window, and placed on the wall surface to the left and right of this opening are two dressed stone date stones, the one on the left of which reads "19," and the one on the right, "12." The center portion is then crowned by a large gable end that is clad in wood shingles.

All of this building's door openings have dressed stone lintels and the school's window openings have both stone lintels and stone sills. In addition, almost all of the building's window openings still retain their original one-over-one-light double hung wood sash windows and the building's original main entrance door and its side and transom lights are also still intact as well."
