Additional Information:Front roof dormer and two side dormers wtih leaded glass windows; rectangular two story bay on north side and three sided two story bay on southside; flat arched windows with brick lintels; full length open porch with short Corinthian columns on brick bases. Elevated foundation. Leaded glass; dentil trim under porch eaves. Identical to 40-34.

Constructed by the Kulzborsky family associated with the Segelke-Kohlhaus Company, this house and the identical adjacent house at 721 South 5th are well preserved, architecturally significant representatives of the American Foursquare house.

One of two nearly identical adjacent houses built by two brothers, Charles and August Kutzborsky.

2017 UPDATE - THIS BUILDING IS SUBSTANTIALLY THE SAME AS WHEN IT WAS LAST SURVEYED IN 1996.
