See record # 0013833 for adjacent institutional building.

Previously surveyed in 1995; listed in the Register in 1999. As of 2007, the adjacent building (AHI#13833) is no longer extant. The address of this building is previously cited as 9508 W. Watertown Plank Road and that is the address the structure is listed in the Register with. It has since been changed to 9480 W. Watertown Plank Road.
